Entry Forms

 EVERY EXHIBITOR, regardless of department or contest, is responsible for filling out the entry form so all necessary information is in the office and checks can be mailed to the correct address in a timely manner.

Please fill in this separate document as well as the back of the registration form.  This sheet is for the livestock superintendents. If you are registering a horse but not a stall, please indicate that on a stall form so the superintendent knows that you did not forget to register for a stall.
